Bureau of Special Education Complaint Information Packet and Form

The Complaint Form is used to submit a formal complaint to the Pennsylvania Department of Education, Bureau of Special Education ( BSE ) - Division of Compliance. This form replaces the Consumer Request Intake form which had been used prior to October 13, 2006.

The link below provides access to important information about the complaint process. You may find this information to be useful before completing the Complaint Form, which can be found at the end of the packet. It must be completed and signed before sending it to the Bureau of Special Education.
