Request Forms

The Office for Dispute Resolution provides several forms to parents, school districts, intermediate units and other agencies to request the various services of ODR. Please refer to the age-appropriate section of either Due Process or Mediation to determine which form you need. The forms are available in PDF (downloadable format) for mailing and/or faxing as well as MS Word versions (for Mediation and Due Process Hearing Requests only) to fill out on line and email to ODR.

English and Spanish versions are available on this site.  Other languages are available from the PaTTAN web site by clicking on Special Education Forms at the top left of the PaTTAN home page.

To print hard copies of the requests and mail or fax to ODR, choose the desired form below:

To fill out a form on line and email to ODR, please follow the instructions below:

1. Save (do not Open) the appropriate MS Word version of the desired form listed below (right-click on the desired form and "Save Target As . . . ."

 -  Due Process Complaint Notice  (MS Word - English)

2. After saving the form, enter all data requested on the form;
3. Save the form again on your desktop and send it as an attachment to an email to: ODR@pattan.net

The other two forms for requesting ODR's services, listed below, must be printed and mailed or faxed to ODR.
